We realize not having health insurance is risky for you and your family. Or you may currently have health insurance, but it is so costly that you cannot afford other things that your family may need. At New Horizon Family Health Services, our Outreach and Enrollment Staff can help connect you and your family to the best health coverage option for you. You may qualify for free or low-cost health insurance! Our team of Certified Application Counselors are available year-round to assist with enrolling for free into a plan through the Health Insurance Marketplace for those that qualify or to apply for Medicaid.  There is no cost or obligation to meet with our staff to find out what is available to you. We will help you understand your options and assist you with the enrollment process.

As of January 31, 2017, the Open Enrollment Period for the Health Insurance Marketplace has passed. However, you may still be eligible for enrolling into coverage!

Do you qualify for the Special Enrollment Period?
Special Enrollment Period is a time outside of the Open Enrollment period during which you and your family have a right to sign up for health coverage. In the Marketplace, you generally qualify for a special enrollment period of 60 days following certain life events such as:

Change in Family Status

Marriage, Divorce, Having a Baby, Adopting a Child, Placing a Child for Adoption or Foster Care

Loss of Health Coverage

Due to: Moving Outside Your Insurer’s Coverage Area, Losing Job-Based Coverage, Divorce, End of an Individual Policy Plan, COBRA Expiration, Aging Off of a Parent’s Plan, Losing Eligibility for Medicaid or CHIP, and similar circumstances. Important: Voluntarily ending coverage does not qualify you for a Special Enrollment Period. Neither does losing coverage that does not qualify as minimum essential coverage.

Changes in Eligibility Status

Gaining Citizenship, Leaving Incarceration, Gaining Status as a Member of an Indian Tribe, (members of recognized Indian tribes can sign up for or change plans once per month throughout the year).
