Late Policy
Patients arriving late for their appointment may be asked to reschedule. This is intended to help decrease the wait time for all patients.
No Show Policy
If you must cancel an appointment, you must call at least 24 hours in advance or you will be considered a no-show. If you have a problem keeping your appointments, please contact us as soon as possible. We may be able to provide assistance so you can keep your appointment or help you reschedule.
A fee of $3.00 will be charged for each no-show for an office visit. A fee of $10 will be charged for each no-show for a scheduled procedure.
After three no-shows, you may be removed from any future scheduled appointments. You will need to call the morning of the day you wish to be seen. We will let you know what may be available at that time.
No Aggressive Behavior
We prioritize the health and well-being of our patients, team members and visitors. Aggressive behavior is NOT tolerated on our premises.
Aggressive behavior includes:
- Physical assault
- Inappropriate contact
- Abusive or foul language
- Threats
Aggressive behavior may result in removal from the facility or arrest.
Unattended Minors
New Horizon Family Health Services, Inc. provides medical, dental, and behavioral health care to patients of all ages. We strive to limit barriers to care while ensuring the safety of our patients. Due to this, the following guidelines are necessary:
- Patients under the age of 16 must be accompanied by an adult to their medical, dental, and/or behavioral health appointments.
- There are specific treatments, procedures, and appointment types that may require an adult to be present for any minor under the age of 18.
- No person under the age of 16 may be left unaccompanied in the lobby.
Service Animal Policy
Service Animals that have been trained to assist a person with a disability recognized under the Americans With Disabilities Act (ADA) are welcomed at New Horizon Family Health Services.
Our staff may ask you:
- If the animal is required to assist you
- What specific task the animal has been trained to perform

All other animals, Including Emotional Support Animals (ESA), therapy, comfort, or companion animals, are not allowed in our facilities for any reason
Handler responsibilities:
- The service animal must remain under control at all times
- The handler must ensure the animal is housebroken and does not cause disturbances
- The handler is responsible for cleaning up after the animal
We want to make sure that you are seen as needed by our medical team. If your animal is not a “Service Animal,” please make arrangements to leave the animal outside of the facility while you are seeing a Provider or, if that is not possible, we are happy to reschedule your appointment to a time when you can make needed arrangements for your animal.
Nominal Fee Increase
Notice of Nominal Fee Increase for Sliding Fee Category A Patients
Effective January 1, 2026
Beginning January 1, 2026, the Nominal Fees for patients in Sliding Fee Category A will increase as follows:
- Medical Services: $15 → $20
- Behavioral Health Services: $5 → $10
- Dental Services: $15 → $50
These changes apply only to Category A nominal fees.
Other sliding fee scales and programs — including pharmacy and the Ryan White Program — are not affected.
Sliding fee discounts are available to eligible NHFHS patients based on household income and family size. Proof of income is required.
