General
New Horizon Family Health Services, a Federally Qualified Health Center (FQHC), provides quality primary care services and access to specialty care commensurate with the needs of the people in the communities we serve. Services are offered regardless of age, race, creed, sex, national origin or ability to pay.
A Federally Qualified Health Center (FQHC) is a reimbursement designation from the Bureau of Primary Health Care and the Centers for Medicare and Medicaid Services of the United States Department of Health and Human Services.
We warmly welcome you to become a new patient at one of our many offices. Please visit our locations page for office services and hours.
Established patients need to bring:
- All prescription medicine bottles
- Insurance card and co-payment amount
- Appropriate fee payment (for “sliding fee” patients)
- Discharge papers from recent hospital visits
- 3 recent pay stubs or notarized letter from employer for household income (for “sliding fee” applicants)
- Insurance cards and co-payment amount
- Appropriate sliding fee payment
- Prescription medicine bottles
- Birth certificate and shot records for children
No. We do not offer free services. A sliding fee discount is available to patients who qualify, based on income and family size. Proof of income required. You may apply for sliding-fee discount at the front desk.

No. Your co-pay covers your office visit only. Your incidentals (i.e. lab work, injections, prescriptions, etc.) are separate charges, but are also discounted.
Medical & Dental
Yes. New Horizon Family Health Services wants to be your medical home to better assure your well being.
Yes. Your appointments are scheduled with the same provider. However, you may change providers.
Each location sees both children and adults.
New Horizon Family Health Services offers extended hours of service to treat illnesses when they strike outside of regular office hours. Our on-call nurse is available to answer any questions you may have after hours. To use this service, please call 864-729-8330. If it is an emergency, however, please call 911.
In the case of an emergency, please proceed to the nearest emergency room.
Call or ask at the office for a same-day sick appointment or “work-in time" at any of our locations. You may also request to be called if there is a cancellation.
We will refer you to the hospital of your choice.
