NHFHS Recognized for Quality Improvement

New Horizon Family Health Services (NHFHS) has received seven Community Health Quality Recognition (CHQR) badges for 2025. CHQR badges are awarded by the Health Services and Resources Administration (HRSA) annually to Health Center Programs and look-alikes that demonstrate exceptional performance and various key areas of Health Center operations.

CHQR badges are awarded based on an analysis of data reported in the annual Uniform Data System (UDS) Report that is submitted annually.  Determinations are made on comparison to national benchmarks, overall best clinical quality score among health centers, improvements in quality score, increase in the total number of patients served and who receive at least one comprehensive service (mental health, substance abuse, dental, vision and /or enabling), services provided to racial/ethnic groups, Electronic Health Record optimization and Patient Centered Medical Home (PCMH) Certification.

NHFHS was recognized as a National Quality Leader in the following areas:

  • Behavioral Health: Screening for depression, brief intervention and follow-up plan
  • Heart Health: Controlling high BP, Statin therapy for prevention, tobacco screening and cessation intervention
  • Diabetes: Adult BMI & follow-up, weight assessment and nutritional counseling, Hemoglobin A1c

NHFHS received community health center quality recognition awards in the following four areas:

  • Improving Access to Care: Increase patient population by 5% and patients receiving mental health, substance use disorder, vision, dental and other enabling services
  • Advancing Health Information Technology (HIT) for Quality: Offer telehealth services, exchange clinical information online with key providers’ health care settings, engage patients through HIT
  • High-Value Care: Based on the growth rate of total costs per patient

In addition, NHFHS was also recognized as a Gold Health Center Quality Leader – one of only six recipients in the state of South Carolina to receive this award. These awards underscore our organization’s commitment to providing quality, affordable, compassionate patient-centered health care to improve the health of the communities we serve.
