What do I need to bring at every appointment? Established patients need to bring: All prescription medicine bottles Insurance card and co-payment amount Sliding fee patients must bring appropriate fee payment Discharge papers from recent hospital visits
New patients need to bring: Picture ID and Social Security Card Sliding fee applicants need 3 recent pay stubs or notarized letter from employer for household income Insurance cards and co-payment amount Appropriate sliding fee payment Prescription medicine bottles Birth certificate and shot records for children
Billing Specific
Can I be seen for free? No. We do not offer free services. A sliding fee discount is available to patients who qualify, based on income and family size.

SLIDING FEE DISCOUNT POLICY
NOTICE TO PATIENTS:
New Horizon Family Health Services serves all patients regardless of ability to pay. However, services are not provided free of charge.
Sliding fee discounts for essential services are offered depending upon family size and income.
Proof of income required.
You may apply for sliding-fee discount
at the front desk.
What are my payment options?
Private Insurance Medicaid Medicare VA Benefits Sliding fee discounts for those without insurance and who qualify. To qualify for the sliding fee scale, bring the 3 most recent paycheck stubs or most recent tax return.
Payment Methods Cash, Check, Visa, Mastercard, American Express, Discover, and money order.
If I am a sliding fee discount, does my co-pay cover all of my visit? No. Your co-pay covers your office visit only. Your incidentals (i.e. lab work, injections, prescriptions, etc.) are separate charges, but are also discounted.
Service Specific
Can I see the same doctor every time? Yes. New Horizon Family Health Services wants to be your medical home to better assure your well being.
Can I change doctors if I want to? Yes. Your appointments are scheduled with the same provider. However, you may change providers.
Can both children and adults visit your locations? Each location, with the exception of the West End Medical Alliance on Pendleton Street, sees both children and adults.
What if I get sick after hours or on the weekend? Our nurse on-call is available to answer any questions you have. To use this service, please call 233-1534 and choose option 4. Then leave a message and a nurse will return your call promptly. If it is an emergency, please call 911.
What if I need to go to the emergency room? In the case of an emergency, please proceed to the closest emergency room.
What if I get sick, but do not have an appointment? Call, or ask at the office, for a same-day sick appointment or “work-in time.” You may also request to be called if there is a cancellation.
What if I need to be hospitalized? We will refer you to the hospital of your choice. If you have no hospital insurance, we have an agreement with the Hospitalists and Teaching Services for Internal Medicine and Pediatrics at Greenville Memorial Hospital to admit our patients.
